​Thank you for your interest in getting involved with Kentucky RHTP, a multi-year, statewide initiative led by the Kentucky Department for Public Health (KDPH) to strengthen access, quality, and sustainability of rural health services and improve the health and well-being of all Kentuckians. We value collaboration and look forward to working with a broad range of partners across healthcare delivery, public health, and community-based organizations as this work progresses.


​​​​​​​​ As of January 2026, the implementation planning phase began, in accordance with the Center for Medicare and Medicaid Services (CMS) grant implementation timeline. At this time, KDPH is not immediately seeking proposals, but will be developing funding opportunities for prospective partners aligned to our approved RHTP application. For additional details on proposed program scope and activities, please refer to the federally approved Kentucky RHTP application and see additional information below for prospective local partners and/or vendors.

Information for Community, Institutional, and Health Partners

RHTP is intended to be implemented in partnership with a range of organizations that contribute to the rural health ecosystem. This includes community-based organizations, advocacy groups, educational and academic institutions, providers, public health entities, and other partners whose work aligns with RHTP goals.


As implementation planning advances, the Commonwealth anticipates identifying opportunities for collaboration that may include program design input, community engagement, workforce development, evaluation and learning, or other aligned activities. Engagement pathways will continue to be refined and communicated as they become available.


For Parties Interested in RHTP-related Procurement Opportunities

Kentucky’s Rural Health Transformation Program is designed to advance long-term, statewide improvements in rural health through coordinated efforts across multiple focus areas. We anticipate engaging qualified vendors and contractors to support program implementation as appropriate, in partnership with providers, community-based organizations, and public health entities, as this work progresses.


Funding related to RHTP must be awarded in accordance with state law, including the competitive procurement process when required. Solicitations will be publicly posted, and links to the solicitation will be posted on the RHT website. Interested vendors are encouraged to monitor the website at ruralhealthplan.ky.gov ​for official opportunities.​
